Symptoms: Sharp, shooting pain, numbness, or tingling radiating down one leg, often worsening with certain movements.


2. Spinal Stenosis

Spinal stenosis, or the narrowing of the spinal canal, typically occurs with age as the spine undergoes wear and tear. This narrowing can compress the sciatic nerve roots, leading to painful symptoms.


Symptoms: Leg pain, cramping, or weakness that worsens with walking or standing and often improves when sitting or leaning forward.

3. Degenerative Disc Disease

Degenerative disc disease results from the gradual wear and dehydration of spinal discs, which act as cushions between vertebrae. Over time, this wear can create nerve compression, especially if bone spurs develop.


Symptoms: Lower back pain with radiating leg pain that worsens with physical activity or sudden movements.


4. Spondylolisthesis

Spondylolisthesis occurs when one vertebra slips out of place over another, leading to potential nerve compression. It can result from fractures, arthritis, or structural imbalances within the spine. Symptoms: Pain radiating down the legs, especially with activities that compress the spine, such as walking or bending forward.


5. Piriformis Syndrome

Piriformis syndrome happens when the piriformis muscle, located in the glute area, tightens or spasms, putting pressure on the sciatic nerve beneath it. This condition can mimic sciatic pain. There are three tests you can do to rule in or out piriformis syndrome vs a herniated disc compressing on the nerve.


Symptoms: Pain and tingling in the buttocks and down the back of your leg, typically aggravated by prolonged sitting or physical activities like running.


6. Trauma or Injury

Sciatic pain can also result from direct trauma to the lower back, such as a car accident, fall, or sports injury. Or like in my case, I hit my head really hard on the ground from doing a front flip 540 in Seattle. You might be wondering how does your neck affect your low back? Pregnancy, especially in the third trimester, can trigger sciatic pain due to weight gain, hormonal changes, and pelvic adjustments that increase pressure on the lower back and sciatic nerve. Conditioning your pelvic floor and inner unit are crucial while in pregnancy to help prevent these types of things from happening if at all possible.


Symptoms: Sciatica-like symptoms, particularly in the buttocks and legs, which may increase with physical activities or prolonged standing.


8. Infections or Tumors

While rare, infections or tumors near the spine or pelvis can also lead to sciatic pain. These conditions may cause nerve pressure and produce sciatica symptoms.


Symptoms: Persistent pain accompanied by additional symptoms like fever, unexplained weight loss, or other systemic signs.


9. Sacroiliac (SI) Joint Dysfunction

Sacroiliac joint dysfunction can result from poor posture, injury, or uneven leg lengths, to name a few, affecting the joint where the spine meets the pelvis. When the SI joint becomes inflamed or irritated, it can cause pain that mimics sciatica. Again, it's important to test and or do X-rays to get a more definitive answer for a proper rehabilitation and corrective exercise protocol.


Symptoms: Pain in the lower back and buttocks that may radiate down the leg, commonly experienced on one side of the body.

10. Spinal Misalignments and Structural Imbalances

Poor posture, leg length discrepancies, and muscle imbalances can lead to spinal misalignments that place undue pressure on the sciatic nerve. Over time, these imbalances can create chronic pain and tension in the lower back. For example you might experience a hypo-lordosis or hyper-lordosis which isn't helpful in preventing sciatic nerve irritation.
